How Do Hair Extensions Work? The Rules, and How Far They Bend

Hair extensions work by anchoring added hair to small sections of the wearer's own hair, set slightly away from the scalp, so that living follicles carry the weight. Five systems dominate: clips that open and close, adhesive tape tabs, wefts sewn onto a braided track, keratin bonds melted around a strand, and metal beads clamped shut. Published manufacturer specifications put the load at each attachment point between roughly 1 gram for a keratin bond and 17 grams for a clip on a high-volume set. That load, multiplied by the hours it stays on, separates a hairstyle from an injury.

Every rule a client is handed — a placement distance, a move-up interval, a gram limit — carries a stated number and an unstated margin. Some have slack in them. One has almost none.

The five attachment methods, and what each one hangs off your hair

The mechanism is identical across methods: a section of natural hair is isolated, added hair is fixed to it, and its follicles become load-bearing. Placement is specified, not improvised — application guidance from the extension supplier Big Kizzy puts the attachment a quarter to half an inch from the scalp: near enough to hide, far enough not to drag at the root.

Laced Hair's published gram guide and spec sheet gives usable figures. Its standard tape-ins ship 20 pieces per pack at 3.8 cm wide, 50 g at 18 inches — 2.5 g per tab. Beauty Works lists its polyurethane tabs at 4 cm and approximately 3 g each. Hand-tied wefts sit in another class: Laced Hair's standard set is 8 pre-cut wefts weighing 113 g, roughly 14 g each. Keratin bonds are lightest per unit, and PA Hair Extensions puts a full head at 100–150 strands.

Divide Cliphair's published Ultra Volume specification — 240–300 g across 8 wefts carrying 18 clips — and each clip holds 13 to 17 g, against 1 g at a keratin bond.

A caveat sits under that arithmetic: a clip's teeth span an inch or two of hair, a weft runs along a track several inches long, a keratin bond grips 25 to 35 strands. Grams per attachment point is not grams per follicle, and nobody publishes the second figure — the one that would predict damage.

Tape-ins or clip-ins for a first set

For a first-time wearer, the deciding comparison is exposure time.

Clip-ins carry the heaviest per-point load in the category, and it matters least, because the wearer takes them out. Cliphair advises no more than 150 g for fine hair, 150–200 g on medium-to-thick hair, and above 200 g only for very thick hair. Overload a clip set and the consequence shows within a day, usually as a sore spot behind one clip.

Tape-ins run 2.5–3 g per tab and stay on for six to eight weeks — roughly a thousand hours of uninterrupted pull, through sleep, washing, and brushing. The load is small; the duty cycle is total. Tape suits a client who wants to stop thinking about the hair. Clips suit one who wants a reversible decision, worth a great deal on a first set.

Why installations break hair

Five mechanisms account for most of the damage, and they compound.

Tension at application. The AAD is unambiguous: pain or a headache while the stylist works means the style is too tight and the area should be loosened before the appointment continues. Shani Francis, MD, MBA, FAAD, a board-certified dermatologist, puts the mechanism plainly in the Academy's guidance: "The constant pulling can cause strands of hair to break or fall out, and it could damage your hair follicles."

Excess weight for the density beneath it. A gram limit is the rule with almost no margin. Fine hair in a 240 g volume set carries a load specified for a different head.

Poor sectioning. Supplier guidance from Doctored Locks notes that an oversized bead needs more base hair to fill it or it slips, and that a bead used in row work should sit about two-thirds full. Sections cut too small put the same weight on fewer follicles.

Delayed maintenance. As hair grows, the attachment travels away from the scalp and gains leverage. Short broken hairs around bonds signal that tension is too high or the move-up overdue.

Rushed removal. Force applied to a bond that has not been chemically released goes straight to the follicle.

What extensions share with a tight braid

Extensions belong in one category with every other style that loads a follicle: tight braids, cornrows, sewn-in weaves, locs, tightly pulled ponytails. The dermatological literature treats them as a single problem, traction alopecia, because a follicle cannot tell a tape tab from a thread. Attachment hardware is the smaller variable.

Nonhlanhla Khumalo, MBChB, FCDerm (SA), PhD, of the Division of Dermatology at Groote Schuur Hospital and the University of Cape Town, led the studies that quantified it. Among 874 African adults in Cape Town, traction alopecia prevalence reached 31.7% in women against 2.2% in men. In a companion study of 1,042 schoolchildren, prevalence was 17.1% in girls, rising from 8.6% in the first school year to 21.7% in the last.

Chemical processing changes the arithmetic. Among those schoolchildren, traction alopecia appeared in 22.9% with a history of braids on natural hair and 32.1% with braids on relaxed hair — a difference the authors reported as not statistically significant in that cohort. The pooled analysis in the Journal of the American Academy of Dermatology in 2008 found the association held, with odds highest when traction was added to relaxed hair: an odds ratio of 3.47 against natural hair.

Two limits belong with those figures. They describe populations — South African, later Cameroonian and Sudanese — where extension and relaxer use is near-universal, so they are not a personal risk score. Khumalo's group also found only 18.9% of those with traction alopecia had never had hairdressing symptoms: four in five had warning first.

What the installation asks of you between appointments

Daily care is unglamorous and non-negotiable. Detangle from the ends upward with a loop brush, holding the hair above the attachment so the pull stops at the hand rather than the scalp. Sleep with the hair loosely braided. Keep conditioner off the bond zone — suppliers warn that residue there traps shed hair and strips the friction beads rely on.

The AAD advises shampooing as often as the hair type requires and protecting the hairline with water-based gels only. Buildup at the scalp is what turns a maintenance appointment into a removal.

Cadence follows the method. Tape-ins are lifted, cleaned of adhesive, re-taped and reset closer to the root every six to eight weeks, and sewn tracks are tightened on the same cycle. Keratin bonds are not moved up — they come out and are replaced at three to four months.

Here the rules genuinely disagree. Manufacturers describe an eight-week cycle repeated indefinitely. The AAD advises wearing a weave or extensions for two or three months at most, then switching to a style without them. Paradi Mirmirani, MD, and Khumalo, writing in Dermatologic Clinics in 2014, went further for traction hairstyles generally: a maximum of two weeks, worn infrequently, completely painless, and on natural hair where possible. Their proposed slogan was blunt — tolerate pain from a hairstyle and risk hair loss.

Nobody in the industry follows the two-week figure. It is still the number a dermatologist would give.

When the complaint stops being cosmetic

A slipping tape tab is a cosmetic complaint. So is a visible bond, or a colour mismatch at the root. Those belong to the stylist who fitted them. A second set of signs belongs with a clinician — and both get reported to the same person, though only one can be fixed with pliers.

Prompt professional assessment is warranted for scalp pain that persists after the installation is loosened; a headache recurring at the same site; pustules, crusting, or bumps at attachment points; visible thinning or recession at the temples or front hairline; short broken hairs forming a band where the traction sits; burning, weeping, or spreading redness; or any bald patch under an installation.

That last has a documented pattern. Jamie Ahdout and Paradi Mirmirani reported in the Journal of the American Academy of Dermatology in 2012 a distinctive horseshoe of hair loss across the occipital and temporoparietal scalp, corresponding exactly to where the patient's glued-in wefts had sat — loss away from the hairline, in the shape of the hardware. Yang, Iorizzo, Vincenzi and Tosti had reported four such cases in the British Journal of Dermatology in 2009, when the literature held only three prior reports.

Khumalo's Cape Town research also recorded traction folliculitis — follicle inflammation from pulling — in 24.3% of the women examined with tightly braided styles, 150 of 604. A stylist can relieve the tension causing it; only a clinician can treat it.

Early traction alopecia is reversible. Scarred follicles are not. That is why the referral threshold sits low.

Taking them out without taking your hair with them

Removal is where the most avoidable damage happens. Resistance means the adhesive has not released, never that more force is needed.

  1. Detangle the length fully, from the ends upward, before anything touches the attachment.
  2. Saturate the bond with the manufacturer's own remover and let it dwell. Published dwell times disagree sharply — some stylist guides specify 30 to 60 seconds, others 3 to 5 minutes, and several product labels give no clock at all, only visible saturation. The formula decides, not the stopwatch.
  3. Slide the released attachment down the hair shaft. Never lift it away from the scalp.
  4. Comb the trapped shed hair out, working the knot loose rather than pulling through it.
  5. Clarify the scalp before any new set is fitted.

Step four alarms almost every first-time client, and it should not. The AAD confirms that shedding 50 to 100 hairs a day is normal; Cleveland Clinic puts the range at 50 to 150. Under an installation those hairs cannot fall away — the attachment holds them. Across a 56-day tape cycle, the AAD baseline gives roughly 2,800 to 5,600 shed hairs released at once. The clump is arithmetic, not loss.

Choosing the next set around a real head of hair

Density sets the ceiling on total weight, and it is the specification worth measuring rather than guessing. Texture sets the method: tightly curled hair carries a documented vulnerability to breakage, which is why the traction literature concentrates on it, and it favours load spread along a track over load concentrated at points.

The cycle cost matters more than the first invoice: a move-up every six to eight weeks indefinitely for tape, two or three full reinstallations a year for keratin, one purchase and no appointments for clips.

Scalp health decides whether any of it proceeds. An installation fitted over active flaking, folliculitis, or a tender scalp seals that problem in for two months.

How do permanent hair extensions work?

No extension is permanent.
